The Blues are believed to be big fans of the Spanish International, and are also keen to offer left back Felipe Luis a return to Atletico, after he failed to impress in his first season at the club.
Koke has been watched by Chelsea's technical director Michael Emenalo, but Atletico are determined to milk as much as possible out of the club.
Man City are eager to land Wolfsburg star Kevin De Bruyne, but The Telegraph say that Bayern Munich are favourites to land the Belgian.
De Bruyne was signed by Wolfsburg for £15m last January, but has impressed for both club and country since, with the club potentially selling him for as much as £45m.
City are believed to be keen on bringing him back to the Premier League, but they say the Bayern Munich look to be the most likely destination for the midfielder, should he leave Wolfsburg.

The Mirror say the 17 goal striker will be sold by QPR in the summer, regardless of whether they avoid relegation or not.
