The former Manchester United captain warned Grealish that he may be on borrowed time at the club after he was an unused substitute in City's latest defeat.
Pep Guardiola's side lost 2-0 to Liverpool on Sunday at the Etihad Stadium, their eighth Premier League defeat of a disappointing season.
The once-all-conquering Man City have been reduced to also-rans, with several of the squad's key players starting to show signs of decline.
Grealish, signed for £100m in 2021, has become a fringe player this season, despite the team's poor results.
The England winger has made only 15 appearances in the Premier League in the current campaign, making just six starts.
The former Aston Villa captain has yet to score in the league this season and has registered only one assist.
Keane believes that Grealish, who turns 30 in September, may be among several City players who leave the club this summer.
"I think Jack is under pressure," Keane said on Sky Sports.
"You know what, Pep's looked at Jack now, he's picking up injuries, his lifestyle and all that type of scenario.
"Look at some big games last year where he (Guardiola) didn't even bring him on.
"I think Jack might be under pressure. Whether Pep wants to move him on, that will be an interesting one.
"The other ones I think are straightforward, the older players 32, 33 - contracts are up and you think that's an easy decision.
"But I think there would be a few players under pressure were it that Pep wants to get rid of them."
Grealish has made 149 appearances for Man City and scored 16 goals. He was a regular starter when the club won the treble in 2023, but has otherwise been a squad player under Guardiola.
Grealish has two years remaining on his contract at the Etihad.
